For appointments you can call directly to the clinics to any of the numbers provided bellow. If you are not able to reach us by phone, you can fill the request form on this page or send an email with your name, phone number, reason for visit and location preference.

For appointments you can send us an email with the following information: Patient Name, Date of birth, Patient's insurance ( MMM Vital, TS, etc), Phone number and Location ( Ponce or Bayamón).
